• Valid photo ID, government issued ID, or Passport*
• Any and all vaccine records **

Dr. Hwang will provide 1-693 form. If you bring your own copy please do NOT sign. It MUST be signed in front of civil surgeon. Whole process will take 4-5 days.


* Alternative proof of indentity may be accepted for children under 14 years of age that shows name, date, and place of birth, parents’ full names, and other identifying information.

** If you don’t have records of vaccination, you will have to get the vaccines again.
